What do you mean "blank"? Blank as in all white or blank as in all fields empty?

If it is all white, take a look in the php error.log
(If you are using "http;//localhost/test/forum", try replacing the ";" with a ":" in "http;//localh...")

I just thought that maybe your SQL is not working offline or better yet  use your computer name in your system property tab if your working from localhost. If you have php admin you can check for connectivity when working offline.

I am connected to a MySQL database, Have PHP 5 installed and running Xitami http server all on my local machine (Windows 2000).

I have declared my host as both localhost and 127.0.0.1, filled in all other required fields (database name, username, password (blank on my local server), admin username and password, admin email and base url)

The values I have used for the base url have been http://127.0.0.1/test/forum and http;//localhost/test/forum.

My problem is that when I click the "Start Install" button I am returned to a blank install.php page and nothing has been set up in MySQL.

Am I missing some information out?
