The odd thing is that mod_rewrite is installed, and the file is correctly named.  hmm  It seems to be throwing a normal 404 error in spite of that.

The htaccess file I got straight from SVN, by the way.

Did you read this?
Important: In order for most URL schemes to work, you need to copy/upload the file .htaccess from the extras directory into the forum root directory. Furthermore, the server that hosts the forums must be configured with mod_rewrite support and must allow the use of .htaccess files

The documentation there isn't complete. If your forum resides in a subdirectory (e.g. example.com/forum), you also need to make a few modifications to the .htaccess. We haven't quite figured out how to instruct users how to do this though.
