In instalation what do I how to write in field where I must put database name. I have no database yet. Is database some file in upload dir? What about username and password for database?

you need to have a mysql or postgres sql database setup or have sqlite setup with or without a database, you can get these details from your host usually

How can get those informations. My website is hosted on one of the free hosting pages who suports php and mysql but I cannot figure it out how to setup a database.

on the control pannel click on manage mysql databases and create the database there
