Registration form - more fields

Hello!

Request is - how to add additional fields to the registration page? I would like this fields (necessarlly required for successful registration):

- name
- location
- date of birth

P.S. Yes, i use "Search", i've seen various topics regarding this, but nobody has written successful instructions on how to do it.
