I just noticed this weird little bug:
The same user seems to be online twice..?
Board version: 1.2.15

My guess: you upgraded to 1.2.15 from an older version and didn't run the database update script

I've had this error a few times, but not often, if i get it only very rarely does it mean i messed up an upgrade or not?

If it still occurs after 1.2.15, you messed up somewhere, yes.
