I'd like to know if there is a way to use PunBB without having to have the /cache directory writeable. My installation of PunBB was hacked yesterday because the /cache directory was writable and they were able to change my cache_quickjump_#.php files (and upload a new file) and I'd rather not take the chance of leaving any directories or PHP files writable.

Works great! I have it up and running on my forums. 
I've also modified the Google Sitemap Generator extension for use with PunRewrite so that the new rewritten URLs are displayed in the xml file. You can download it from http://www.punres.org/desc.php?pid=149.
