I just figured it out.  I tried to wipe out the old folder and then upload the new one instead of overwriting the old folder with the new one.  I didn't realize that there were files in the old folder (like the config file) that the update code needs.

I'm having the same problem.  I'm trying to upgrade from 1.2.9 to 1.2.14.  My forum is in a subfolder off the root of the website (e.g. http://www.mysite.com/forum).  I put the 12_to_1214_update.php file in that folder and it won't work.  Are we expected to install the forum into the root of site?  I guess I could use http://forum.mysite.com.

Bibby, how did you get this to work in the end?

I don't think they're spam bots.  I'm not getting any spam posts.

The lurkers theory is interesting.  I would say my ratio of posters/non-posters is also roughly 60/40.

I get a fair few people (maybe 2 a week) registering on my site, but then not making a post.  I can't quite figure out why and I'm curious.  Does anyone have any ideas?

I'm in the same position as knight01.  I use punbb for a support forum which is used infrequently.  I probably only get 1 new topic a month, so I often miss new topics.  Are there still plans for a subscribe to forum feature?
