What you're asking for could certainly be written as an extension though.

Nope, it's the correct behavior, it doesn't note that an edit has taken place at that point: it doesn't wipe out the fact that previous edits have occured. I'll see if I can make the wording explain that a little better.

Queries are not a measure of speed. Take a look at the two page generation times you just quoted, for example, and notice that PunBB is close to 30x faster.
You can have slow queries and you can have fast queries. I could run a query that would take as long as 1000 general queries.
When you have a lot of them and they take a lot of your page generation time, then it's time to look at optimizing them.

And the index page isn't necessarily a good comparison if you're talking about modifications: there isn't some kind of correlation between queries on the index page and modifications. For example, on the index page you linked to I see one query's worth of data that PunBB doesn't include (recent posts). Otherwise, it's the same.

Yes, it's part of <pun_footer> in the main template. The code for it is generated in footer.php

Well, you could grab a copy of 1.3 (which does support UTF-8) from SVN and test that if you wanted, it would be useful to get more feedback on it from a PostgreSQL/UTF-8 perspective. You shouldn't be using that on a production site though, since it's beta software.
Otherwise, I'm not really sure. Adding /u to preg_* functions isn't the only change required to get PunBB 1.2 working properly with UTF-8.
