OK then, download a clean copy of PunBB and replace all the files (except for config.php, of course)

Plugins are on PunBB, unless they're a part of a mod, in which case they're on Punres with the rest of the mod
And I'm not sure how the installer works, but how I'd assume it works is to take the readme and, based on a general format, do the changes from there

since it's 5mb, the DB plugin won't let you upload it
phpMyAdmin has the ability to load those files from directly off the server: I would look into that

I haven't tested any of them, but they would make multiple entries for the same user id/ident impossible
