punbb_users is because you setuped table prefix in config or in install.
Config.php is auto-generated - why you made the config.php file?
Seems like you remove table prefix settings from config or use invalid config.php.

Im checked version 0.6.8 - it work on MySQL and PunBB 1.4.
You shure that you use 0.6.8 version?
Check webserver error logs.

Check server logs for errors.
Check that forum db user have rights for alter table.
CHeck rights for extensions/pun_attachment/attachments/ef75594baec7b6a90151f4815bd0d46b folder.
We do not support this extension PunBB in 1.4.

You can update last visit click by link mark all topics as readed - this link update your last visit.
