I believe it would be a good idea to have a single function that returns the proper HTML code to display an avatar, and have a hook in that function.
This would allow extensions to modify the displaying of avatars, for example adding image filetypes.

Wouldn't all the columns (including headers) be much more malleable if they were stored in an ordered numbered array rather than as associative array?

In the current version, which is 1.2, it indicates the forums/topics which have been posted in since your last visit.
This has been changed in 1.3.

If an extension was properly designed, the link to the page will be in the correct section with the other options for standard features. It's alot better than the old plugin system IMO. You just have to change your mentality a bit.

If it doesn't use XmlHttpRequest, it isn't AJAX. It's Javascript, or if you prefer the very old name, DHTML.

Try using a basic text editor. I'm not sure what it is on OSX. Don't forget to save as non-UTF8 plain text.
