Im not experimented with Engine type.
Im use MyISAM 5 years without any problem on http://forum.iteam.ua but make backup every night.

In a case of MySQL DB im recommended use OPTIMIZE and ANALYZE query at least once per week (better every night). Many hostings with CPanel do it automatically.

Cant reproduce on punbb 1.3.5 and pun_attachment 1.0.3 with any redirect timeouts.

Cant reproduce - login works.
Seems like cache issue, maybe proxy.
All works fine, except broken/missed css files from pun_tags ext.

You can write in CSS files rules for adding icons as background for this IDs.

Its simple — your code send data BEFORE headers. Maybe its just few chars saved in file or something.
